Adejoke Sijuwade, CRNP, FNP-BC, NP-C is a board-certified Family Nurse Practitioner with credentials from both American Nurses Credentialing center (ANCC) and the American Academy of Nurse Practitioners (AANP). With over a decade of diverse clinical experience spanning Critical care, Primary care, Endocrinology and Military settings, she brings unparalleled expertise and a deep commitment to patient-centered care to Cliniva Primary Care, LLC.

Adejoke specializes in managing acute, non-acute and chronic health conditions, effectively addressing a wide spectrum of illnesses such as hypertension, Diabetes, Hyperlipidemia, Asthma, COPD, Men’s Health and Women’s health. She is especially passionate about Diabetes management, preventive health education, and promoting wellness through a holistic approach.

 

At Cliniva Primary care, Adejoke fosters a compassionate and supportive environment. Her patient care philosophy centers on collaboration, empowering individuals to actively participate in their health journey. She creates personalized care plans tailored to each patient’s unique needs, helping them achieve and maintain optimal health outcomes.

 

When not caring for patients, Adejoke enjoys reading, cooking, meditating, travelling and volunteering. She looks forward to welcoming new patients to Cliniva Primary care and is dedicated to providing expert care to individuals and families in the community.
